HHA/CNA Requirements:
- C.N.A. License or H.H.A Certificate
- Driver’s License
- Social Security Card
- Auto Insurance
- Proof of High School diploma
- Physical (Stating that you are free of communicable disease, dated within the last 6 months)
- PPD/TB Skin Test dated in the last 12 months, or Chest X-Ray in the last 12 months if history of positive PPD
- Applicable Immigration or Legal Status (if it applies)
- CPR/BLS Card
- Assistance with Self Administered Medications Certificate (dated within last 2 years)
- Alzheimer’s Certificate (dated within last 2 years)
- HIV/AIDS (dated within last 2 years)
- OSHA Certificate (dated within last 2 years)
- Domestic Violence Certificate (dated within last 2 years)
- Liability Insurance (dated within 1 year)
- A copy of your Resume
- For the 5 Certificates, you can obtain online at www.ceufast.com if you do not already have them.
- For Professional Liability Insurance, you can obtain online at www.cmfgroup.com
- COVID Vaccinated
